OES with demodectic mange

My 13 yo sheepdog was diagnosed with demodectic mange. I started him on a low dose of 1% ivermectin and he had no reaction. After 4 weeks with no improvement, I raised the dose to .5 ml/day. After a few days he became extremely weak and unable to walk. I immediately stopped the treatment and he improved the next day. I did not give him anything else but his mange symptoms began to improve dramatically. It has been 6 weeks since the incident and he seemed to be doing well. Now I noticed that his mange appears to be coming back. I am afraid to give him the ivermectin again, so is there something else that I could try?

Has his thyroid been tested? Just wondering if hypothyroidism might be contributing to the mange problem. (Best to find the cause if possible rather than just treating symptoms.) Definitely a question for the vet along with whether ProMeris would be a safe option if he responded poorly to Ivermectin...
